TO ALL EXAMINEES under the 𝗕𝗔𝗖𝗛𝗘𝗟𝗢𝗥 𝗢𝗙 𝗦𝗖𝗜𝗘𝗡𝗖𝗘 𝗜𝗡 𝗣𝗛𝗔𝗥𝗠𝗔𝗖𝗬 program– here is the 𝗟𝗶𝘀𝘁 𝗼𝗳 𝗣𝗮𝘀𝘀𝗲𝗿𝘀 for the 𝗨𝗖𝗨 𝗖𝗼𝗹𝗹𝗲𝗴𝗲 𝗔𝗱𝗺𝗶𝘀𝘀𝗶𝗼𝗻 𝗧𝗲𝘀𝘁 (𝗖𝗔𝗧), 𝗙𝗶𝗿𝘀𝘁 𝗦𝗲𝗺𝗲𝘀𝘁𝗲𝗿 𝗔.𝗬. 𝟮𝟬𝟮𝟱-𝟮𝟬𝟮𝟲.

As you prepare for enrollment, kindly ensure you have the following documents ready:

1. Form 138/137
2. Certificate of Good Moral
3. Barangay Clearance
4. PSA copy of Birth Certificate (photocopy only)
5. Passport size Picture (3pcs)
6. Medical Certificate with results of the following: Chest X-ray, CBC, and Urinalysis
7. Two (2) Long Brown Envelope

1. Official Transcript of Records (OTR)/Copy of Grades (if OTR is not yet available)
2. Honorable Dismissal/Transfer Credentials
